Kindergarten Registration
Your child must be 5 years old by September 1, 2010. You will need to bring
the following: a certified birth certificate to be copied by the office (not a hospital document) and proof of
immunizations.
Idaho State law requires all students, preschool through 12th grade, to show documentation of
immunity upon enrollment in school or have a signed exemption on file or the child will be denied attendance.
Required
Immunizations:
DTaP - 5 doses, or only 4 doses if the 4th dose was given after the child's 4th birthday
MMR -
2 doses
POLIO - 3 doses or more doses
HEPATITIS B - 3 doses
Please make sure your child's
immunizations are up to date. Immunizations are given at Payette Lakes Medical Clinic daily from 9 - 11:00 a.m.
and 1 - 4:00 p.m. Monday through Friday. You may call them at 634-2225, if you have further questions.
Immunizations are also given at Central District Health only by appointment. Please call them at 634-7194 for more information.
Please remember to bring your child's records with you to Kindergarten registration.
